On April 15, 2008, the Centers for Medicare & Medicaid Services published updated Conditions for Coverage for End-Stage Renal Disease Facilities in the Federal Register (available to view at the following location:
http://www.cms.hhs.gov/CFCsAndCoPs/downloads/ESRDfinalrule0415.pdf). These regulations modernize Medicare’s ESRD health and safety Conditions for Coverage and update CMS standards for the delivery of safe, high quality care to dialysis patients. The revised regulations are patient-centered, reflect improvements in clinical standards of care, require the use of more advanced technology, and outline a framework to incorporate performance measures viewed by the scientific and medical communities as related to the quality of care provided to dialysis patients.

Electronic Data Submission Requirement
The final rule requires electronic submission of administrative and clinical data by all Medicare approved dialysis facilities in the United States (§494.180(h)). This requirement takes effect on February 1, 2009. To support this mandate, CMS announces CROWNWeb, a web-based software application that all dialysis facilities will be required to use beginning February 1, 2009. Use of CROWNWeb for data submission beginning February 1, 2009 is mandatory to support CMS’s goals of quality improvement and performance assessment, as well as to ensure prompt claims processing and reimbursement.

CROWNWeb’s Relationship to the 2008 ESRD Conditions for Coverage
The CROWNWeb application provides direct support for the Final Rule of the 2008 Conditions for Coverage in multiple ways. The following items illustrate CROWNWeb’s this support and includes the relevant section and/or page numbers from the 2008 ESRD Conditions for Coverage

• CROWNWeb provides a mechanism for complying with the electronic data submission requirement
(section 494.180(h))
• CROWNWeb offers technology that protects data in accordance with HIPAA regulations (p. 20443)
• CROWNWeb collects and verifies CPM data from 100% of the patients at contributing facilities
(p. 20470)
• CROWNWeb enables comparative reporting for a facility against Network/National aggregate data
(p. 20442)
• CROWNWeb supports the 1995 Paperwork Reduction Act (p. 20454)
• CROWNWeb users may choose from two types of CROWNWeb training (instructor-led or web-based
training) which will be provided nationwide to facility staff and other users (p. 20468)
CROWNWeb System Requirements

To access CROWNWeb, a facility must have a computer connected to the Internet that meets the minimum specifications outlined in the table on the next page. Most computers built within the last four years will meet these minimum requirements. Additionally, high-speed Internet connectivity (DSL, cable, or faster) is required.

CrownWeb Need to Know!
As of 2/1/2009 every dialysis facility has to maintain its own forms and enter the forms (CMS-2728/CMS-2746) into CROWNWeb. Before you have access to the system, a few things have to be done.

  1. You have to have designated a System Administrator (SA) and Back up SA.
  2. You need to complete the registration form for the SA and mail it to Network 13.
  3. You have to have a user account for the system.
  4. Use Project CROWNWeb for online training.

System Administrator (SA) and User Accounts
The Network will create all the Facility-SA accounts, but Facility-SA will be the person that creates users in your facility. This person may/may not be a person that enters forms into the system. All facilities have to have an SA and backup SA. You can start now by designating an SA and completing the required Registration FORM. Mail the registration form to the Network (USPS, return receipt required).

The system administrator (SA) will create users in the facility. SA may serve as a user. Registration Form for User is the same as the SA registration form, but SA has to complete and enter the form in to the QIPS system.
