The reports and information on this Web site have the ability to be downloaded and copied from your computer. Official documents and reports may be requested by contacting the Network with your request. There may be a charge for particular items to cover all costs. When making your request, be specific as to what you require. Please contact the Network if you would like official reports by e-mailing, calling or writing to the following:

Network 13 Annual Report

The Network 13 Annual Report is done every spring and completed in June. Information in the annual reports consists of Network accomplishments for the year. Topics include the structure of the network, network goals and activities, and statistical data on the network's facilities and patients. For convenience, we converted each section and subsection to pdf format for downloading and printing. Sections contain all the contents within the subsection. Sub-sections only contain its components. (Updated July 26, 2010)

Dialysis Facility Report (DFR)

Release of the 2010 Medicare Dialysis Facility Reports (DFR)
from University of Michigan, Kidney Epidemiology and Cost Center

This year the distribution of the DFR is web-based only. User accounts (email address and passwords) are needed to access the DFRs online and the comment submission portion of www.DialysisReports.org. To add new user accounts, master account credentials are required.
The Network provided credentials for initial log in for all facilities on June 22, 2010.  Please remember, your report is available to you only on the secure Web site www.DialysisReports.org.
All user accounts created last year remain in effect. If you have the master account information from 2009 or created user account(s) last year, you do not need any additional information from the Network in order to do access your 2010 information once it is posted.  Returning users who have forgotten passwords may enter a username (email address) and click on “FORGOT PASSWORD” to reset a password.
Please note that the public Web site includes a FAQ page with helpful log-in information. It has instructions on how to log in, what to do if you forget your password, and how master account holders can create access for authorized users.
You may submit comments from July 12, 2010 through August 18, 2010 at the secure Web site www.DialysisReports.org. As before, the comments will be reviewed to determine if any measures should be suppressed on the DFC website. Comments for your State Survey Agency (SSA) will be appended to your facility’s report prior to making the report available to the SSA. Should you have additional questions, please contact the DFR line at 734-998-9823 or send an email to the DFR mailbox at keccdfr@umich.edu.

What is the Dialysis Facility Report?

Guide to 2009 Dialysis Facility Reports: Overview, Methodology, Interpretation
The Dialysis Facility Report is conducted each year by the University of Michigan Kidney Epidemiology and Cost Center (UM-KECC) with funding from the Centers for Medicare & Medicaid Services (CMS).  The Report is based on data from Medicare dialysis and hospitalization claims, Medical Evidence Forms (CMS-2728), Death Notification Forms (CMS-2746), Annual Facility Survey Reports (CMS-2744), other CMS data and Social Security Administration data. 

It includes updated quality measures that were posted on the Dialysis Facility Compare (DFC) website in November 2009 and information that was sent to your State Survey Agency (SSA) in September 2009 for the State’s use in survey activities.

Please note that these data could be useful in quality improvement and assurance activities.  The information contained in this report facilitates comparisons of patient characteristics, treatment patterns, transplantation rates, hospitalization rates, and mortality rates to local and national averages.
