Checkout Policy
Many items on the following list do not have to be returned, but videos*, audiotapes*, books*, and some publications* are only available to borrow for 30 days. There is a two video limit per check out. The borrower is responsible for the cost of returning the video(s) or book(s). If the borrowed materials are damaged or are not returned by the specified date, the borrower will be sent an invoice for the cost of the replacement. You may request the checkout form by calling ESRD Network 13 at (405) 942-6000 or by filling out the Material Checkout Form pdf-464Kb and sending it to ESRD Network 13.
